Quality Improvement Manager

Position SummaryThe Quality Improvement Manager manages and implements efforts at Y-Haven to ensure that quality improvement (QI) and performance management (PM) activities are developed and managed using a data driven focus that sets priorities for improvements aligned to ongoing strategic initiatives.
Job Responsibilities Working with QI team and Y-Haven's leadership team, creates, coordinates and implements the annual quality improvement (QI) plan.
Plan includes core measurements and the process for gaining client input.
Develops reports and prepares statistical information to meet internal and external reporting requirements and schedules.
Oversees Y-Haven QI process and ensures agency compliance with billing (Medicaid, etc.
) standards, licensing regulations, records, files and reporting requirements.
Maintains Y-Haven standards to achieve highest level of organizational accreditation (i.
e.
, CARF) by studying new and existing requirements, and developing systems for evaluation methods and control.
Participates in the development and implementation of QI policies and procedures including preparing and revising manuals, guidelines and other directives.
Chairs and facilitates Y-Haven's QI Committee, identifying committee functions, descriptions of accountability, roles and responsibilities.
Conducts program reviews to evaluate service delivery and program compliance with all standards of care as outlined by internal and external sources, i.
e.
, agency policies and procedures, OMHAS, CARF, ADAMHS Bd.
, OHS, and makes recommendations based on analysis of same.
Assists in preparation for CARF audits.
Creates various reports for presentation to the Quality Improvement Committee and Y-Haven's Advisory Board.
Oversees the annual client satisfaction survey process.
Ensures that tools are collecting items relevant to client and program needs.
Analyzes data, makes recommendations to programs, shares survey results and recommendations with clients.
Oversees agency response to subpoenas, summons and information requests from Law Enforcement including communication with agency legal counsel and outside attorneys, coordination with staff , fulfillment of records and video requests.
Oversight of HIPAA-compliant information exchanges between outside entities, and coordination of archiving and retrievals with Iron Mountain.
Conducts HIPAA Site audits with QI team, compiles results and provides to Directors for remediation of identified issues Qualifications Bachelor degree in social work, counseling or a social service related field, 1 year of experience, and least one of the following state licenses:
LPC, LCDC-III or LSW.
Or, Bachelor degree with minimum of two (2) years work experience in social service environment with auditing/compliance experience.
CPR and First Aid certification and Safety training required within 90 days of hire.
