Who we're looking for:
We're looking for a Physical Therapist to provide physical therapy services in local assisted living facilities, as well as in our patients' homes! This position requires travel in the Cleveland, OH area - including Lorain, Cuyahoga, Medina and Summit counties.
Responsibilities:
The Physical Therapist is responsible for providing physical therapy services such as start of care oasis visits, reassessments, implementing plan of care, completing related documentation and coordinating with other treating professionals to ensure quality patient care.
Qualifications/
Experience:
Ohio PT License
Current CPR
Experience in a Home Health setting preferred
Valid drivers' license and vehicle insurance, and access to a dependable vehicle.
