Social Worker- Program Coordinator (Dementia Care)

The incumbent serves as program coordinator for the Dementia Care Coordination Program, an outpatient case management and care coordination program. He/she will function as a member of Dementia Care Coordination Team, under Medicine Service within the Geriatrics Section. This specialty treatment program is designed to provide preventive and proactive continuous patient-centered dementia care interventions Duties include, but are not limited to:
Facilitate and promote the use of dementia care coordination. Works collaboratively with both existing VA providers/programs and community organizations to optimize accessibility and utilization of resources and to integrate delivery of health care services for those patients with dementia and their caregivers. Responsible for participating in program development and program implementation. Manages/ plans and monitors the delivery of services for the DCC Program and is accountable for clinical program effectiveness and modification of service patterns. This includes program evaluation and planning for expansion of services/staffing based on service needs. Work Schedule:

Social Workers must possess a full, active, current, and unrestricted license or certification in a State, Territory, or Commonwealth (i.e. Puerto Rico) of the United States or in the District of Columbia to independently practice social work at the master's degree level. Social Workers appointed to direct patient care positions must be proficient in both spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). GS-12 Grade Determination:
One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
